Licensed Practical Nurse (LPN)

Serenity Hospice Care is seeking a compassionate, organized, and detail-oriented Licensed Practical Nurse (LPN) to join our North Office team. This is an excellent opportunity for an experienced LPN who enjoys supporting clinical operations in an office setting while helping ensure exceptional care for our hospice patients.

The In-Office LPN provides clinical and administrative support to the interdisciplinary hospice team. This role assists with coordinating patient care, reviewing documentation, communicating with field clinicians and providers, and ensuring compliance with hospice regulations and company policies.

• Provide clinical support to the Director of Nursing, RN Case Managers, and interdisciplinary team.
• Review patient charts for completeness and regulatory compliance.
• Process physician orders and ensure timely signatures.
• Assist with medication profile reviews and documentation.
• Coordinate communication with physicians, pharmacies, facilities, and referral sources.
• Answer clinical phone calls and appropriately triage patient and family concerns.
• Assist with admissions, recertifications, and discharge documentation as needed.
• Monitor incoming laboratory reports, orders, and clinical documentation.
• Maintain accurate electronic medical records (EMR).
• Support quality assurance and performance improvement initiatives.
• Assist with scheduling and coordinating patient visits when necessary.
• Participate in interdisciplinary team (IDT) meetings.
• Ensure compliance with CHAP, Medicare, and hospice regulations.
• Perform additional duties as assigned to support office operations.

• Current New Jersey Licensed Practical Nurse (LPN) license in good standing.
• Minimum one (1) year of hospice experience preferred.
• Experience with hospice EMR systems preferred.
• Excellent communication, organizational, and time management skills.
• Ability to prioritize multiple tasks in a fast-paced environment.
• Strong computer skills, including Microsoft Office and electronic documentation systems.
• Professional, compassionate, and patient-centered attitude.
