LPN Med Care Manager

• Administer medications and treatments per physician orders
• Follow the Six Rights of Medication Administration
• Document in MAR/TAR accurately and in real time
• Monitor symptoms and administer medications as ordered for symptom relief
• Report refusals or adverse reactions promptly
• Conduct Service & Health Updates (SHUs)
• Transcribe physician orders and support care plan updates
• Observe and report physical, emotional, or cognitive changes
• Maintain medication carts, complete narcotic counts, and uphold compliance standards

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• Experience in senior care or medication administration preferred

Pay Range: $31.90 - $42.60 per hour.

In addition to base compensation, Sunrise may offer discretionary and/or non-discretionary bonuses. Eligibility to receive such a bonus will depend on the employee’s position, plan/program offered by Sunrise at the time, and required performance pursuant to the plan/program.
