Registered Nurse – Hospice

$3100.80/week -

Job Summary:

Provides comprehensive hospice nursing care to patients in their homes, assessing needs, developing care plans, and delivering compassionate support to patients and families. Ensures the safe and effective management of patient care within regulatory guidelines and organizational standards.

Responsibilities:

• Perform thorough patient assessments to determine appropriateness for hospice care, learning needs, and progress toward goals.
• Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team.
• Complete admission and re-visit documentation promptly, ensuring data transfer within specified time frames.
• Educate patients and families using age-appropriate teaching methods, evaluating and revising teaching plans as needed.
• Manage case responsibilities by prioritizing patient needs, evaluating and revising care plans based on evidence-based practices.
• Ensure safe medication management, including medication reconciliation and maintaining current medication orders, while adhering to pharmacology standards.
• Participate actively in interdisciplinary team meetings, providing updates on patient progress and revising care plans.
• Maintain a safe environment by following infection control protocols and reporting infections appropriately.
• Contribute to performance improvement initiatives by reviewing health records, supporting quality activities, and mentoring new staff.
• Assist in growing the agency by participating in orientation, cross-training, and other assigned duties.

Qualifications:

• Minimum of 2 years of RN experience in a home hospice setting.
• Experience with electronic medical records (EMR) systems.
• Strong critical thinking and excellent communication skills.
• Valid New Jersey RN license and a current NJ driver’s license.
• BLS certification required.
