Registered Nurse – Hospice

$2422.50/week -

Job Summary:

Provides specialized hospice nursing care to patients in their homes, ensuring comfort, safety, and adherence to care plans. Manages patient assessments, education, and interdisciplinary communication to support quality end-of-life care.

Responsibilities:

• Perform comprehensive patient assessments to determine appropriateness for hospice care and progress toward goals.
• Develop, implement, and evaluate individualized care plans based on evidence-based practices and patient needs.
• Ensure timely documentation of admission, re-visit, and discharge assessments within established time frames.
• Educate patients and families using age-appropriate methods, documenting teaching tools used and revising plans as needed.
• Supervise home health aides and coordinate interdisciplinary team activities, including bi-weekly meetings.
• Maintain a safe environment by following infection control protocols and managing medication safety through routine reconciliation and current medication orders.

Qualifications:

• Must have at least two (2) years of experience as an RN in a home hospice setting.
• Demonstrate proficiency with Electronic Medical Records (EMR) systems, critical thinking, and strong communication skills.
• Current RN licensure in New Jersey.
• Valid New Jersey driver’s license and BLS certification.
• Knowledge of hospice regulations, insurance reimbursement guidelines, and pharmacology.
