Virtua seeks a dedicated Hospice RN Case Manager to deliver compassionate end-of-life care within the New Jersey hospice service area. You will assess needs, educate families, and coordinate with an interdisciplinary team to provide comfort, symptom management, and support at patients’ homes statewide.
Required are NJ RN licensure, 2–3 years hospice experience, CPR/BLS certification, and a valid driver's license. This role involves on-call, weekend, and holiday coverage as part of the care team.
