Bright Star Care of Clinton is hiring a full-time Home Health RN to provide skilled nursing care to patients throughout Hunterdon County and surrounding areas.

Requirements:

• Current NJ RN license or multistate/compact RN license allowing practice in NJ

• Minimum 2 years of home care/home health experience

• Current BLS and ACLS

• Reliable transportation

• Must be comfortable performing independent home visits

• Strong assessment and clinical skills

Responsibilities:

• Perform comprehensive nursing assessments during home visits

• Obtain and document patient history

• Provide wound care and infusion therapy

• Administer and manage medications

• Follow physician orders and plans of care

• Monitor patient condition and report changes

• Educate patients and families

• Coordinate care with physicians and other healthcare professionals

• Complete required clinical documentation
