Provides primary medical coverage, performs initial and semi-annual assessments, performs additional physical assessments of participants as needed and develops and implements appropriate plans of care to Inspira LIFE program participants. Evaluates participant physical complaints and provides appropriate treatment. Provides participants and caregiver teaching and education. Functions as a member of the Interdisciplinary Team (IDT). Directs daily activities of Inspira LIFE Clinic.
Rounds in the Inspira LIFE contracted nursing facilities. Regularly informs the IDT of the medical condition of each participant, remaining alert to pertinent input from other team members, participant’s caregivers, as well as documenting changes in a participant’s medical record consistent with documentation policies. Performs clinical on-call duties on a rotation basis with the other primary care providers. May need to provide position support at another Inspira LIFE Center due to operational need.
As a multisite provider of PACE community-based services for seniors in Cumberland, Gloucester and Salem Counties, an Inspira LIFE employee may work at either or both the Vineland or Williamstown LIFE Program as census changes or operational needs occur to meet the needs of our participants.
Education & Experience:- Graduation from a master's-level program at an accredited school of nursing.
- Minimum of 2 years working with the frail or elderly, Geriatrics Nurse Practitioner and Spanish Speaking Preferred.
- RN License – New Jersey.
- APN License – NJ State Board of Nursing.
- Certification as a Nurse Practitioner with prescriptive authority.
- Controlled Dangerous Substance (CDS) and DEA required.

- Requires corrected vision and hearing to normal range. Must be able to communicate verbally with all staff, vendors, and clients. Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment. Requires lifting/carrying up to 65lbs. and pushing or pulling 150lbs. using appropriate body mechanics and equipment.
- Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Provides primary medical coverage, performs initial and semi-annual assessments, performs additional physical assessments of participants as needed and develops and implements appropriate plans of care to Inspira LIFE program participants.
- Integrates the primary care treatment plan into the overall plan of care developed by the interdisciplinary team. Interacts with team members to meet emergent and acute needs of participants. Participates in discharge planning for acute and long-term placement.
- Functions as a member of the interdisciplinary team. Maintains regular attendance at and participates in interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for 24-hour care delivery.
- Evaluates and treats participants during acute illness. Manages participants’ chronic illnesses and conditions.
