Tri County Care Management Organization (TCCMO) is a non‑profit organization that provides care management services for children with complex challenges in Hunterdon, Somerset and Warren Counties.

We are currently seeking a Care Manager Specialist based in our Branchburg, NJ office to support care management activities while learning and practicing the core values of the wraparound process. This is a full‑time position.

Responsibilities
  • Assist Care Managers who provide intensive case management services to families and children with behavioral, emotional, social, developmental, and mental health challenges.
  • Provide transportation to youth and families.
  • Act as primary backup for Care Managers during absences.
  • Travel to various meetings at homes, schools, courts, and attend other services with the youth and families as needed.
  • Connect families to therapeutic and community services.
  • Complete progress notes, service plans, and other required documentation in electronic records according to agency standards.
  • Assist other agency departments as needed.
Qualifications
  • Bachelor’s degree in social work, psychology or related discipline.
  • Strong interpersonal and organizational skills.
  • Bilingual skills desirable.
