Home Care Supervisor

Currently offering a $2,500 sign on bonus for this position.

Lower Burrell, PA.

Schedule
: 7:30 a-4 p M-F with on call rotation + as needed based on participant care.

The Home Care Supervisor (HCS) will provide consistent supervision to Community Care Aides using effective communication and guidance. This person will strengthen the relationship between the Home Care Department and the Inter Disciplinary Team (IDT). This person will provide hands on leadership in all aspects of home care. The HCS will develop partnerships with families and caregivers. The HCS may be required to make in-home visits as necessary for assessment and on‑going case management.

This position will participate in weekend/on‑call rotation.

• Accredited nursing program for LPN or CNA.
• Program approved by the National Board for Certification in Occupational Therapy.
• Physical Therapy Assistant program approved by the American Physical Therapy Association.

5 years of direct care experience with at least 1 year of experience working with the frail and elderly population. 2 years of supervisory experience. Experience teaching patients and other health care workers preferred.

Valid Pennsylvania COTA/L, PTA, CNA or LPN license. Valid PA Driver’s License.

• Ability to work with the interdisciplinary team approach to care for the elderly.
• Working knowledge of local health care and geriatric service networks.
• Basic working knowledge of Windows operating systems, e‑mail, word processing.
• Able to deliver services in a compassionate, responsive, and courteous manner.
• Able to work effectively with staff, participants, providers, and referral sources.
• Interest in geriatrics and community‑based programming.
