Position: Outpatient Nurse (LPN)

Job Purpose/Summary

The Outpatient Nurse provides nursing services to consumers and nursing support to medical prescribers in the outpatient setting. The incumbent utilizes medical and behavioral health skills to assist with removing medically related barriers to progress, using a professional nursing process to provide a holistic approach to nursing interventions. The position promotes mental health and physical well-being throughout treatment and crisis intervention.

The Outpatient Nurse administers, dispenses, and safeguards medications stored at the agency centers and performs nursing duties in accordance with standard agency practices and nursing license guidelines.

Minimum Qualifications

Education: Nursing license by the Commonwealth of Pennsylvania (LPN).

Experience: Minimum of 6 months of experience working in a psychiatric setting preferred. Experience with child, adolescent, and adult psychiatric nursing and/or substance abuse preferred.

Licenses, Certifications & Clearances: Act 33/34 Clearances; FBI Clearance;
Valid State of PA Class 1 Driver’s License.

  • Knowledge of federal and state regulations and requirements relative to psychiatric/behavioral health nursing
  • Critical observation, assessment, and reporting skills
  • Knowledge of available community resources that may be needed as additional supports
  • Knowledge of Microsoft Office and electronic medical record (EMR) keeping
  • Ability to travel locally between work sites
  • Conducts mental health and physical health assessments, as needed and according to ANA standards of nursing practice, with appropriate follow-up
  • Provides consultation/education to consumers, family members, and other staff via phone or in person
  • Develops and maintains a treatment plan that reflects consumer needs and goals; reviews and updates the plan with multidisciplinary treatment as per department regulations
  • Prepares and administers medications as ordered by the agency MD and per agency policies; assists in safeguarding medications
  • Assists medical prescribers with ordering diagnostic tests and assessing need for medication refills and potential adverse reactions
  • Monitors laboratory results; informs prescriber of abnormal results or health issues; assists with medical follow-up arrangements
  • Completes any needed prior authorizations for prescription medications and/or treatments
  • Supports the development and implementation of the Medication Assisted Treatment Program
  • Acts as a consultant between behavioral health and physical health providers to improve coordination of care
  • Assesses crises and responds to agency crisis codes with appropriate action
  • Adheres to all privacy regulations and laws
  • Complies with nursing licensure regulations and requirements
  • Maintains clinical records; documents contacts and updates records/forms; reviews charts for compliance with regulations
  • Completes trainings, stays current with safety standards, and uses universal precautions when administering medications or providing direct care
  • Provides coverage for nursing services as directed by supervision
  • Acts as a consultant for medical issues within nursing education
  • Adheres to PA regulations and OSHA/Department of Transportation requirements for management of waste
