Position Overview
We are seeking a compassionate and experienced Registered Nurse (RN) Case Manager to provide high-quality hospice care. In this role, you will coordinate and deliver patient-centered care in collaboration with an interdisciplinary team (IDT), ensuring comfort, dignity, and quality of life for patients and their families.
This position involves regular travel to patient homes within the assigned territory.
Key Responsibilities
- Conduct comprehensive initial and ongoing patient and family assessments to determine hospice care needs
- Develop, implement, and update individualized Plan of Care in collaboration with the IDT and physician
- Provide education and emotional support to patients, families, and caregivers
- Deliver and coordinate nursing care, including symptom and pain management
- Monitor patient conditions, including nutrition, hydration, and disease progression
- Supervise LPNs and home health aides; complete required supervisory visits and documentation
- Participate in interdisciplinary team meetings and quality improvement initiatives
- Respond to patient needs, including participating in on-call rotation (approx. 3-4 days/month)
- Provide end-of-life support and complete discharge documentation as required
- Ensure compliance with all regulatory standards, policies, and infection control procedures
- Maintain accurate, timely, and compliant clinical documentation
Qualifications
- Active Registered Nurse (RN) license in the state of Pennsylvania (in good standing)
- Minimum 1-2 years of nursing experience (hospice, oncology, medical-surgical, or related preferred)
- Strong clinical skills in symptom management, bedside care, and patient/family support
- Ability to work effectively within an interdisciplinary team
- Excellent communication and documentation skills
- Valid driver's license, reliable transportation, and auto insurance
