Position OverviewThe Rehabilitation Counselor (RC) supports the Rehabilitation Specialist (RS) in assisting individuals with severe and persistent mental illness to develop or restore independent living and social skills. The RC facilitates decision‑making regarding life, self‑care, illness management, and community participation, and promotes access to community resources to enable integration, recovery, and prevention of relapse and re‑hospitalization.
Primary Function- Plan, manage, and coordinate the PRP.
- Collaborate with RS and PD to implement programs, set goals, and recommend organizational policies.
- Ensure staff compliance with standards and regulations.
- Educate staff on documentation, goal setting, and IRP/DLA
20 procedures. - Prepare for upcoming regulatory changes (e.g., RS COMAR).
- Assist in managing all PRP site locations and facilitate statewide travel to program sites.
- Seek opportunities for local, state, and federal funding.
- Prepare and submit budget proposals to RS.
- Maintain service quality standards and submit expenditures per budget policies.
- Coordinate rehabilitation functions with information services.
- Collect data for qualitative and quantitative service needs comparison.
- Develop outreach strategies and maintain referral pipelines.
- Build relationships with key stakeholders (behavioral health associations, therapists, hospitals).
- Represent the organization in public and community relations.
- Attend staff and committee meetings and supervise employees.
- Participate in public education and relations within service area.
- Support CARF accreditation preparation and ongoing compliance.
- Engage in professional development opportunities.
- Assist clients in gaining interpersonal skills.
- Document client symptoms and interventions to meet client needs.
- Collaborate with treatment teams to develop client interventions.
- Provide social–skills training to improve communication and interpersonal behavior.
- Support clients in community reintegration (behavior control, social skills, ADLs).
- Design interventions and services for client success.
- Offer feedback to the treatment team for complex clients.
- Arrange community services and collaborate with treatment teams.
- Assist in clinical care planning, discharge paperwork, after‑care, and placement.
- Facilitate or co‑facilitate client groups.
- Complete progress notes and document all contact with agencies.
- Contribute to treatment planning sessions.
- Complete three (3) visits each month for minors and six (6) visits each month for adults.
- Conduct a mix of in‑home/community and office visits to provide individual and group services.
- Submit all client visit documentation on time.
- Submit monthly documentation by COB the day before month end.
- Submit Treatment Plans every 90 days (review).
- Engage in monthly supervision with direct supervisor.
- Attend staff meetings and training each month.
- Collaborate with client therapist, psychiatric provider, and other providers for goal continuity.
- Coordinate bi‑weekly with parents, guardians, community providers, court representatives, teachers, and others to monitor behavior and address barriers.
- Provide guidance on vocational, educational, family, social, and daily life choices.
- Link clients to resources and assist with daily organization responsibilities.
High School Diploma; B.A / B.S. Preferred. Experience or education related to mental illness and client care is required. Strong interpersonal communication and client‑centered skills.
Preferred SkillsUnderstanding of general psychosocial standards and medical practices for mental illness treatment. Bilingual and experience in a mental health setting is preferred.
