Job Description:As a Home Health Nurse, you will provide high-quality, patient-centered care in a home setting. You will collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.

Responsibilities:

• Provide skilled nursing care to patients in their homes
• Perform assessments, monitor vital signs, and administer medications
• Educate patients and families on managing medical conditions and treatments
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Maintain accurate and timely documentation in compliance with regulations
• Evaluate and adjust care plans as needed

Qualifications:

• Valid Registered Nurse (RN) license
• Minimum of 1 year of clinical nursing experience (home health experience preferred)
• New graduates are encouraged to apply
• CPR Certification
• Strong communication and organizational skills
• Ability to work independently and manage time effectively
• Proficiency in electronic medical records (EMR) systems
