Prescribes, coordinates, and delivers care through the nursing process. Maintains the standards of professional nursing. Supervises CNA and coordinates care with other health care workers as assigned.
Provides comprehensive initial assessments to all clients and on subsequent visits continually assesses physical, psychosocial, environmental and community resources available utilizing the principles of growth and development over the life span.
Initiates Plan of Care, re-evaluates the need for care and modifies Plan of Care as ordered/needed. Establishes discharge plans with patient, physician, other disciplines and caregivers.
Completes OASIS data.
