• Minimum of 1 year of recent LPN experience
• Active Connecticut LPN license
• Current BLS certification
• Eligibility to work in the United States
• Valid driver’s license and reliable transportation
• Physical exam within the past year
• PPD or Quanti
  
  FERON within the past year, or chest x-ray within five years if previously positive
