Intake Licensed Practical Nurse; LPN or Registered Nurse; RN - Hybrid

Position: Intake Licensed Practical Nurse (LPN) or Registered Nurse (RN) - Hybrid
This candidate must reside in New Hampshire.

This is a full time, hybrid position with work performed remotely (in NH) and/or in the New London office. The ideal candidate is self-motivated, tech-savvy and an excellent communicator.

We are looking for a team player who is willing to jump in and help, is flexible, able to prioritize and stay organized. A detail oriented Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our Intake Department full-time. This person will assist with both Homecare and Hospice referrals.

Responsibilities for Intake Licensed Practical Nurse (LPN) or Registered Nurse (RN):

* Accurately enter patient referral information into the electronic medical record system

* Accurately enter all incoming patient care orders and notify appropriate team members as necessary

* Navigate multiple electronic referral software portals

* Communicate with discharge planners, physician offices and liaisons regarding patient referrals while maintaining positive relationships

* Triage all incoming phone calls

* Communicate effectively with patients and patient care givers

Requirements for Intake Licensed Practical Nurse (LPN) or Registered Nurse (RN):

* Active NH Licensed Practical Nurse (LPN) or Active NH Registered Nurse (RN) License

* Must reside in New Hampshire

* 2 years of Health Care experience

* Knowledge of medical terminology, familiarity with a medical office setting, and/or triage

* Knowledge of Home Health/Hospice

* Must be comfortable working in a remote environment

* Proof of Flu vaccination

* Reliable transportation

* Valid driver's license

* Automobile insurance

* Physical exam within the last year

* Two-step TB test
