We are seeking a compassionate and skilled Registered Nurse (RN/LPN) to join our interdisciplinary team. As a Registered Nurse (RN/LPN) at Beechwood Post Acute & Transitional Care, you will have the opportunity to work with a dynamic team of healthcare professionals and provide high-quality care to our residents. Your responsibilities will include:

• Delivering patient-centered care
  :
  Assess, plan, implement, and evaluate individualized care plans for residents in a post-acute and transitional care setting.

• Collaborating with the interdisciplinary team

  Work closely with physicians, therapists, social workers, and other healthcare professionals to ensure comprehensive care.

• Maintaining accurate records
  :
  Document resident care and treatment plans in accordance with regulatory requirements and facility policies.

• Participating in quality improvement initiatives
  :
  Contribute to ongoing quality improvement efforts to enhance resident satisfaction and outcomes.
