HoursRegular Full-time, Overnight Weekend Friday, Saturday, and Sunday
ShiftsOn-Call after hours 4:30pm to 8:00am Friday, Saturday, and Sunday
SalaryGuaranteed 30 hours of pay

- Ability to triage all agency patient (home health and hospice) related phone calls
- Complete all patient communication related documentation utilizing patient EMR
- Collaborate and communicate effectively with other team members/program managers
- Provide home health and hospice visits in patient homes and facilities
- Travel throughout entire service area Other duties as assigned
- Current NH Registered Nurse (RN) license
- Minimum 2 years of health care experience required
- Home Care experience is preferred but not required; we will train the right candidate
- Telephone triage experience preferred
Additional requirements include reliable transportation, valid driver's license, automobile insurance, current physical exam, TB testing, and required vaccinations.
