St. Joseph Residence is seeking a Full-Time Day Shift - Monday through Friday MDS Coordinator
Requires 1 week per month as RN on-call
St. Joseph Residence is a not-for-profit Catholic organization seeking caring, compassionate, and responsible nurses to join our direct care clinical team at our 50-bed skilled nursing facility, providing compassionate care in a faith-based environment, supporting and promoting resident and patient independence, respect, dignity, and community.
The role of MDS Coordinator requires someone with a minimum of one year of experience in MDS, Rn assessments and care planning.
Key Responsibilities
Assessment & Documentation:
Coordinate the MDS process with the interdisciplinary team and complete assessments, MDS entry, and care plans, utilizing resident, family, and staff interviews, physical exams, and medical record and physician order review. Care areas include pain, falls, hydration, psychotropic medication use, skin breakdown, catheter use and elopement risk.
Care Planning:
Develop and update care plansupon admission/discharge, change of condition, and quarterly for all residents, ensuring updated care plans, measurable goals and assigned services.
Quality & Safety:
Conduct floor rounds, supervise and educate staff, attend care conferences, review 24-hour reports, and monitor changes in resident conditions. Assist in quality checks and training to ensure care is provided by staff according to the care plan.
Reporting & Analysis:
Distribute monthly quality indicator reports and assist Director of Nursing and quality assurance committee on identifying and implementing performance improvement projects.
Education & Training:
Train staff in MDS processes, policies, regulatory updates, care processes and competencies.
Interdisciplinary
Collaboration:
Attend Medicare meeting, care conferences, and assist social services in determining discharge planning needs.
Restorative Program:
The MDS nurse is responsible for overseeing the restorative nursing program, ensuring restorative goals are individualized, resident-centered, and evaluated and updated on an ongoing basis.
RN On-Call:
The MDS nurse is a leadership position required to participate in on-call rotation one week per month. It may be necessary for the MDS nurse to work the floor, participate in training and supporting new staff, and to assist the nurse leadership team in ensuring nursing department support and staffing is provided as needed.
Requirements
Current RN license in the State of Wisconsin
Current CPR certification or ability to obtain prior to employment

Required:
