Current certification from abem or aoa em or acgme or aoa post grad program or other appropriate credentials - must have state medical license prior to credentialing process begins (in the event of urgent situation can submit app) nm csr and dea with nm address. Acls and pals certification (atls is not required). Procedure logs - 24 months required. 3 peer references, immunizations, current flu, phs requires the first two vaccines-monovalent 2 doses or one bivalent dose or completion of covid 19 vaccination form (if available- not required) and department chairperson reference.

Abps/nbpas is not an acceptable board certification at dan c. Trigg hospital.
