The Case Manager In Training role is a full-time, on-site position located in Eunice and New Iberia, LA. The individual in this role will support senior case managers by learning to assess client needs, document care plans, and coordinate non-medical personal care services. Daily tasks include assisting with intake and service planning, maintaining accurate records, communicating with clients and their families, and collaborating with caregivers and office staff.
The Case Manager In Training will observe and participate in home visits, monitor service quality, and help ensure that client care complies with company policies and state requirements. This role provides structured training and mentorship to develop case management skills and prepare for increased responsibilities over time.
- Ability to develop rapport with clients and families, demonstrating empathy, patience, and strong interpersonal skills.
- Organizational and time management skills to handle multiple cases, documentation requirements, and follow-up tasks effectively.
- Clear written and verbal communication skills for documenting care plans, preparing reports, and coordinating with caregivers and staff.
- Basic computer proficiency, including use of email, electronic records, and office software for scheduling and documentation.
- Commitment to client-centered care, confidentiality, and adherence to company policies and state regulations.
- High school diploma or equivalent required; additional education in social services, healthcare, or related fields is preferred.
- Experience in personal care, home care, or human services is beneficial but not required; willingness to learn in a structured training program is essential.
- Reliable transportation to travel locally for home visits and appointments, and ability to work a consistent full-time schedule on site.
