Location: San Ysidro
Essential Functions
- Hire, supervise, lead, and coordinate the PACE Day Center staff including Floor Supervisor, Companionship Coordinator, CNAs, PCAs, and reception staff.
- Ensure recruiting, selecting, orienting, and training of employees, maintaining a safe, secure, and legal work environment and providing personal growth opportunities.
- Communicate job expectations to employees to achieve results; manage employee performance through coaching, counseling, and disciplining; conduct performance evaluations for the nursing team; prepare work schedules and ensure employees have necessary resources to succeed.
- Coordinate all‑day center activities, overseeing staff in the implementation of care plans.
- In collaboration with the PACE Center Manager and Food Services Supervisor, coordinate services and activities offered in the day center, ensuring compliance with state regulations and appropriate staffing ratios; ensure staff are assigned to carry out CACFP duties such as meal delivery and meal counts.
- Oversee the quality of care and safety of PACE participants at the Day Center.
- Serve as a liaison between disciplines, administration, and direct staff.
- Coordinate with the marketing department to promote PACE services in the community and host welcoming visits for new candidates at the center.
- Ensure execution, maintenance, and coordination of subcontracted services.
- Coordinate all activities related to new participants.
- Manage and work within the PACE Day Center budget, communicating regularly with the finance department about operations and needs.
- Proactively engage with PACE Administration to support program development, including participation in working committees and project development/implementation.
- Become a super user of the PACE Electronic Health Record (EHR) system and, in collaboration with the IT Department, ensure initial staff training, ongoing proficiency, and appropriate use of the EHR.
- Review monthly operation expenses to ensure budgetary compliance and identify any budget variations as appropriate.
- Participate in budget planning processes and reviews.
- Ensure compliance in all areas of program operations and prepare the necessary Corrective Action Plan (CAP) as needed in a timely manner (CMS, DHCS, Clinic, Dept. of Education, DHCS Office of Long Term Care).
- Investigate, monitor, and track all PACE participant grievances and incidents to ensure prompt resolution of their issues.
