Job Responsibilities
  • Provides direct patient care as defined in the State’s Nurse Practice Act
  • Implements plan of care initiated by the registered nurse
  • Provides accurate and timely documentation consistent with the plan of care
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor
  • Uses equipment and supplies effectively and efficiently
  • Participates in personal and professional growth and development
  • Performs other duties as assigned by the registered nurse
  • Maintains verbal and/or written communications among other personnel involved in the care of the patient; maintains clinical records on all patients referred to physical therapy and in consultation with the licensed physical therapist
  • Performs and submits required documentation of patient’s clinical notes, progress reports, summaries and case conferences along with changes in orders
  • All patient visit notes must be turned in the next scheduled work day, not to exceed forty‑eight (48) hours
  • Advances professional growth; participates in training sessions at least two times annually; attends short courses, and studies professional journals and texts
Qualifications
  • Licensed Vocational Nurse in the state where they are working
  • Good communication skills
  • CPR certification current
  • Valid driver license
  • Ability to work well with others, function well under pressure, and function independently
  • Must comply with the standards outlined in the Nurse Practice Act
  • Acts only within the scope of their state license or state certification or registration
  • Maintains qualifications current at all times
  • Current State License to practice in New Mexico as a Licensed Vocational/Practical Nurse
  • Bilingual English/Spanish preferred
