Why the job exists

The LPN Wound Care Supervisor exists to coordinate and oversee wound prevention, monitoring, and treatment services within the facility, supporting nursing operations by ensuring skin integrity, wound healing, and infection prevention to promote resident safety, regulatory compliance, and quality care. The role involves managing wound care assessments, treatments, and documentation; supporting pressure injury prevention and skin integrity initiatives; supervising and coaching staff;

assisting with education and competency validation; providing direct resident care when necessary; ensuring regulatory compliance and participating in quality improvement efforts; collaborating with interdisciplinary teams to monitor resident progress; and supporting nursing operations during staffing shortages or increased demand.

How it gets done (Responsibilities)

Conduct comprehensive wound and skin assessments for residents. Develop, implement, and oversee individualized wound care plans. Ensure timely and accurate performance of wound treatments according to provider orders and facility policies. Monitor wound healing progress, document outcomes meticulously, and adjust care plans as needed. Support pressure injury prevention efforts, including turning, repositioning, and nutritional interventions. Supervise, coach, and support Certified Nursing Assistants and nursing staff to ensure compliance with best practices in skin care, infection control, and safety.

Assist with staff education, competency validation, and orientation programs to promote ongoing professional development. Provide direct resident care when necessary, including administering medications, supporting transfers, and assisting with daily activities. Participate actively in interdisciplinary team discussions, wound rounds, and care plan reviews to monitor resident progress and address concerns. Ensure compliance with regulatory standards, participate in audits, and support survey readiness activities.

Support quality assurance and continuous improvement initiatives related to wound care and resident safety. Assist with nursing operations during staffing shortages or increased care demands by stepping into direct care roles and supporting resident safety and care continuity.

Customers

The primary customers are the brave men and women that answered our nation's call, executed their duties and now are unable to live in their respective communities due to failing health. This job also supports the widows of the service men and women, including parents that have lost children in combat (Gold Star Parents).

Ideal Candidate

The ideal candidate is a licensed Practical Nurse with experience in long-term care or skilled nursing environments. They possess strong wound care knowledge and preferably hold wound care certification or are able to obtain one within the first year of employment. The candidate should demonstrate excellent supervisory and coaching skills, with the ability to lead staff, support education, and promote best practices.

They are detail-oriented, highly organized, and capable of managing multiple priorities. A compassionate team player with effective communication skills, they are committed to resident safety, quality care, and regulatory compliance.

Minimum Qualification

Licensure as LPN or LVN, and two (2) years of experience as it pertains to the essential duties and responsibilities of the classification. Any combination of education from an accredited college or university in a related field above licensure requirements and/or direct experience in this occupation totaling two (2) years may substitute for the required experience.
