Responsibilities- Administer medications on schedule
- Monitor vital signs and perform routine assessments
- Assist with activities of daily living, wound care, and charting progress in electronic health records
- Collaborate closely with RNs, physicians, and support staff to develop individualized care plans and identify potential health concerns early
- Communicate clearly with residents and families
- Advance practice through continuing education, mentorship, and evidence‑based care initiatives
