A 13-week local travel home health RN assignment in Albuquerque, New Mexico. Key Responsibilities:Deliver individualized in-home nursing care according to care plans
Conduct comprehensive OASIS assessments and document findings
Administer medications, monitor vital signs, and manage wound care and chronic conditions
Educate patients and families on treatments and disease management
Coordinate care with interdisciplinary team to ensure continuity
Document care accurately using Well Sky/Kinnser EMR
Travel within designated area and receive mileage reimbursement
Active New Mexico RN license or valid Compact RN license
Current AHA BLS certification
Minimum 1 year of recent home health nursing experience
OASIS documentation experience
Valid driver's license, reliable vehicle, and auto insurance
Skills:
Experience with Well Sky/Kinnser EMR
You hold an active NM RN license and BLS certification. You have at least one year home health experience with OASIS. You are comfortable using Well Sky Kinnser EMR and traveling
