RN Clinical Educator

The RN Clinical Educator serves as a clinical resource for patients, providers, and healthcare staff, promoting high-quality, patient-centered diabetes care throughout the Primary Care Department. In collaboration with Primary Care, Diabetes, and Endocrinology providers, the Clinical Educator develops and delivers individualized patient education to support evidence-based diabetes management. The RN-Clinical Educator helps to enhance patients' self-management skills and contributes to quality improvement initiatives aimed at improving clinical outcomes and reducing diabetes-related complications.

This specialized RN Clinical Educator for Diabetes care will be required to:

• Provide individualized and group diabetes self-management education for patients with Type 1 and Type 2 diabetes.
• Educate patients on medication administration, insulin therapy, continuous glucose monitoring (CGM), blood glucose monitoring, nutrition, exercise, and lifestyle modification.
• Assess patients' educational needs, health literacy, readiness to learn, and barriers to successful disease management.
• Collaborate with primary care physicians, advanced practice providers, pharmacists, dietitians, care coordinators, and other members of the healthcare team to develop comprehensive care plans.
• Monitor patient progress and reinforce treatment plans through follow-up visits, telephone outreach, and care coordination.
• Assist providers with patient education related to new diabetes technologies, medication changes, and chronic disease management.
• Document patient encounters and education in the electronic medical record in accordance with organizational and regulatory standards.
• Maintain current knowledge of diabetes management standards, medications, devices, and clinical practice guidelines.

Must be capable of taking the Certified Diabetes Care and Education Specialist (CDCES) after training and preparation.

General duties include:

• Assists with the coordination, development, implementation, and evaluation of clinical education in the assigned clinical areas.
• Serves as a resource to nursing staff, nursing students, and other healthcare personnel in the acquisition of knowledge and skills related to nursing competency and critical thinking.
• Collaborates with Department of Nursing Education & Professional Development and unit department director/manager to oversee clinical training and patient safety initiatives in the assigned clinical area.
• Models excellence in nursing practice through the utilization of advanced concepts in the areas of assessments, diagnosis, planning, implementation, and evaluation of perceived, actual, or potential problems that occur in a variety of patient populations.
• Provides oversight of all new clinical practices and reinforces current practices and processes with clinicians in the assigned clinical areas, establishing competency when appropriate.
• Collaborates with clinical staff nurses, unit directors/managers, Department of Nursing Education & Professional Development, and other members of the health care team to assure clinician performance meets established evidence-based standards; ensuring patient safety and quality is maintained.
• Demonstrates effective leadership:

• Complies with all hospital and departmental policies, procedures and programs.
• Participates in departmental and hospital committees and meetings, and special projects as assigned to facilitate improvement of patient care and development of goals and objectives for the unit's patient population.
• Maintains current knowledge, generates new knowledge, and integrates evidence-based practices into all educational initiatives.
• Participates in and supports research activities.
• Participates in and supports quality improvement initiatives.
• Introduces and supports new ideas; embraces meaningful change; communicates the vision for the change; develops programs to educate staff and help staff cope with change; provides leadership in the professional practice setting and the profession; demonstrates energy, excitement, and a passion for quality work.

Experience qualifications include:

• 2 to 7 years of experience in clinical settings/Registered nurse.

Education qualifications include:

• Required:
  
  Bachelor's Degree Nursing.
• Preferred:
  Master's Degree Nursing Education.

Licenses and certifications include:

  Basic Life Support Health Care Provider.
