Veteran Health Education Coordinator Registered Nurse

The Veteran Health Education Coordinator Registered Nurse is responsible and accountable for all elements of the nursing process when providing and/or supervising education. Delivers a fundamental understanding of Veteran-centered health education and adult learning principles and is a lifelong learner that can practice with flexibility to adapt or change learning strategies based on the assessment of health education and learning needs within the healthcare organization.

Assumes responsibility for the coordination of education focused on patient education, self-management, healthy living, and customer satisfaction throughout the healthcare continuum.

Supports a coordinated approach to planning, delivery, and evaluating evidence-based Veteran-centered health education programs, services, and resources. Follows procedures per established policies and guidelines.

Influences educational outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in the department level quality improvement process and initiatives as well as customer service programs.

Assesses, plans, implements, and evaluates educational needs to facilitate learner development and skill training to help Veterans treat and prevent health conditions and optimize their whole health, well-being, and quality of life.

Facilitates recommendations for health education to improve offerings to Veterans and their families, so they can access and appropriately use Veterans Health Administration (VHA) health care resources across the continuum of care.

Forecasts Veteran health education needs based on trends in the Veterans Affairs (VA) and in the community.

Develops entry level coaching and mentoring skills to support Veteran self-management and well-being. Systematically evaluates current practice, formulates outcomes for groups of patients and/or organizational processes within area of expertise.

Reviews current nursing literature for new trends and changes based on evidence-based research with the ability to propose practice and documentation revisions based on evaluation of outcomes.

The VHEC demonstrates leadership in delivering and improving holistic care through collaborative strategies with others.

Leads the provision of nursing care to patients with complex care needs utilizing the nursing process in alignment with the ANA Standards of Nursing Practice.

Communicates and collaborates as an effective leader of the interprofessional team, at the expert level, with improved evidence-based and/or data-driven outcomes for the immediate practice setting.

Expands knowledge base and practice evaluation through mentoring, precepting, and/or teaching to improve outcomes in the immediate practice setting.

Protects printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, federal regulations, VA statutes and policy, and VHA guidelines. Protects the data from unauthorized release or from loss, alteration, or unauthorized deletion.

Follows applicable regulations and instructions regarding access to computerized files, release of access codes, etc., as set out in the computer access agreement that the incumbent signs.

Pay:
Competitive salary, regular salary increases, potential for performance awards

Work Schedule:

Monday - Friday 07:30am - 04:00pm, subject to change based on the needs of the facility.
