In this role, you will work collaboratively with physicians, care teams, and community partners to coordinate care, remove barriers to treatment, and ensure seamless patient experiences across the continuum of care. Position Summary

The LPN Patient Navigator functions as an integral member of a multidisciplinary healthcare team, serving as an advocate, interpreter, educator, counselor, and clinical liaison for patients. This role coordinates patient care from diagnosis through treatment and survivorship, facilitating communication among patients, families, providers, and support services.

The Patient Navigator provides expert nursing guidance, patient education, care coordination, and resource management while ensuring continuity of care and supporting optimal patient outcomes.

What You'll Do

Patient Advocacy & Care Coordination
  • Coordinate written and verbal communication regarding treatment plans among patients, families, physicians, and healthcare team members.
  • Ensure continuity of care by collaborating with internal departments, external agencies, and community resources.
  • Serve as a primary point of contact and advocate for patients throughout the care continuum.
  • Follow patients through inpatient and outpatient settings, collaborating with care management teams as needed.
  • Help patients and families navigate healthcare systems and overcome treatment-related barriers.
Clinical Support & Navigation
  • Assist patients in understanding diagnoses, treatment plans, and available resources.
  • Review medical documentation to support complete and accurate treatment planning.
  • Facilitate appointments and referrals for:
    • Physicians and Specialists
    • Social Workers
    • Physical Therapy (PT)
    • Occupational Therapy (OT)
    • Speech Therapy (ST)
    • Dietitians
    • Financial Assistance Resources
    • Other supportive care services
  • Provide guidance and support during difficult healthcare decisions.
Patient & Family Education
  • Educate patients and caregivers regarding disease processes, treatment plans, expected side effects, and community resources.
  • Assess educational needs and document teaching activities appropriately.
  • Promote informed decision-making through clear, compassionate communication.
  • Ensure education is continuous, comprehensive, and tailored to individual patient needs.
Quality & Program Support
  • Assist with collecting data, tracking outcomes, and supporting service line initiatives.
  • Participate in strategic planning and quality improvement efforts.
  • Review, update, and develop policies and procedures that enhance patient care and professional practice.
  • Maintain organized workflows and prioritize responsibilities effectively.
Community Engagement
  • Serve as a resource for community education programs, health fairs, screenings, symposiums, and lectures.
  • Support staff education and professional development activities.
  • Build relationships with community partners to enhance patient access to healthcare services.

Qualifications

Education

Graduate of an accredited Practical Nursing program.

Licensure & Certifications

Current Licensed Practical Nurse (LPN) license through the Louisiana State Board of Practical Nurse Examiners

Current Basic Life Support (BLS) for Healthcare Providers certification through the American Heart Association
