Provide comfort when patients need it most.
The Care Team Hospice is seeking two Full-Time Night Shift Licensed Practical Nurses (LPNs) to join our New Philadelphia team. This unique role offers the opportunity to provide compassionate bedside support for hospice patients during overnight hours while working alongside our experienced On-Call Registered Nurses.
You ll care for patients who are actively transitioning, require overnight symptom management, or need additional support when family caregivers or facility staff need assistance. On nights with fewer urgent patient needs, you ll also provide routine hospice visits, creating a rewarding role that offers both variety and meaningful patient care.
Schedule- Full-Time, Hourly Position
- Night Shift
- Approximately 3–4 nights per week (final schedule based on candidate availability and operational needs)
Based out of our New Philadelphia office, serving patients throughout:
Territory will be divided for 2 LPNs.
- Tuscarawas County
- Coshocton County
- Holmes County
- Carroll County
- Harrison County
- Provide one-on-one bedside care during life s most important moments
- Support patients, families, and caregivers overnight when they need it most
- Work closely with experienced hospice RNs and the interdisciplinary team
- Opportunity to perform routine hospice visits when patient needs allow
- Help expand our Continuous Care services while improving the patient and family experience

- Provide skilled nursing care under the direction of the Registered Nurse while following each patient s individualized plan of care.
- Monitor patient conditions, recognize changes in health status, and communicate clinical updates to the care team.
- Assist with symptom management, medication administration, treatments, and patient comfort measures.
- Complete accurate and timely documentation of patient care and observations within the electronic medical record (EMR).
- Collaborate with physicians, Registered Nurses, caregivers, and the interdisciplinary team to deliver compassionate, high-quality hospice care.
- Active Licensed Practical Nurse (LPN) license
- Previous nursing experience required; hospice, home health, long-term care, or skilled nursing experience preferred.
- Strong clinical assessment, communication, and patient care skills.
- Valid driver s license, reliable transportation, and current automobile insurance.
- Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
