As a Clinical Manager, you'll partner with branch leadership to oversee daily clinical operations, support patient care coordination, mentor field clinicians, and ensure compliance with hospice regulations and documentation standards. You'll serve as a clinical resource for your team while collaborating with physicians, referral sources, patients, and families to deliver exceptional end-of-life care.

Key Responsibilities:
• Lead and support daily clinical operations while ensuring compliance with hospice regulations, company policies, and quality standards.
• Mentor, educate, and develop clinical staff through coaching, orientation, documentation review, and ongoing performance support.
• Oversee patient care coordination, care plan reviews, admissions, orders, and interdisciplinary team collaboration to ensure exceptional patient outcomes.
• Monitor quality metrics, conduct chart audits, and support survey readiness and performance improvement initiatives.
• Partner with branch leadership, physicians, referral sources, and operational teams to promote clinical excellence and outstanding patient and family experiences.
Qualifications:
• Active Registered Nurse (RN) license required.
• Minimum of two years of RN experience, including at least one year of leadership experience in hospice, home health, or a related healthcare setting.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong leadership, clinical assessment, communication, and organizational skills.
• Ability to coach teams, manage multiple priorities, and thrive in a fast-paced healthcare environment.
• Valid driver's license, reliable transportation, and current automobile insurance required.
