This Licensed Practical Nurse (LPN) position plays an integral role on the clinical team by providing safe, evidence-based care to patients and residents. The LPN supports clinicians, anticipates the needs of patients and colleagues, and helps maintain a comfortable, organized, and hygienic care environment. Responsibilities
- Monitor, document, and communicate patients' vital signs, statistics, and measurements in a timely and accurate manner.
- Observe patients closely and promptly note any physical or emotional changes, escalating concerns to appropriate clinicians as needed.
- Prepare examination rooms for patient visits, ensuring all required equipment, supplies, and documentation are ready before each appointment.
- Select, prepare, and manage medical supplies and equipment, maintaining proper inventory levels and ensuring items are in good working order.
- Maintain a patient environment that is comfortable, organized, safe, and hygienic, adhering to infection control and safety protocols.
- Maintain accurate and up-to-date records of patient information, including assessments, interventions, and outcomes, using electronic medical records (EMR).
- Prepare patients for follow-up care after treatment by providing clear instructions and ensuring they understand their care plans.
- Potentially guide and educate other LPNs by sharing best practices, supporting onboarding, and serving as a role model for quality care.
- Serve as a knowledgeable resource on nursing-related issues within the unit or section, assisting colleagues with questions and clinical processes.
- Organize and manage scheduling for patients and necessary medical tests, coordinating with the broader clinical team to support efficient patient flow.
- Demonstrate consistent attendance and reliability to support continuity of care and team effectiveness.
- Contribute to a respectful, collaborative, and professional work environment focused on patient safety, high standards of care, and staff satisfaction.
Essential Skills
- Active Licensed Practical Nurse (LPN) license in the state of New York (NYS LPN License).
- At least 2 years of experience working as an LPN.
- Proficiency with electronic medical records (EMR) systems.
- Experience in a healthcare setting providing direct patient care.
- Strong attention to detail in monitoring, documenting, and communicating patient information.
- Ability to recognize and report physical and emotional changes in patients promptly.
- Organizational skills to manage examination rooms, supplies, and patient schedules effectively.
- Commitment to attendance and reliability to ensure consistent patient care.
- Ability to maintain a safe, hygienic, and comfortable environment for patients and residents.
- Strong communication skills to collaborate effectively with clinicians, patients, and team members.
Additional Skills & Qualifications
- Experience working in a long-term care, rehabilitation, or similar resident-focused environment.
- Comfort using EMR systems for documentation and coordination of care.
- Ability to guide and educate other LPNs and support staff when needed.
- Strong interpersonal skills to build meaningful relationships with residents and co-workers.
