Key Position Details:
- .8 FTE (64 hours per two-week pay period)
- 8-hour day/evening shifts from 12:00 PM - 8:30 PM (day off to be determined)
- No weekends, no holidays
- Occasional on-call
- Travel is required within our territory
- Eligible for mileage reimbursement at the IRS rate
Job Description:
Nursing is the diagnosis and treatment of human response to actual or potential health problems. This includes establishing an intentional therapeutic relationship between a registered nurse and a patient and family. As a leader and the integrator of care, the professional nurse has the responsibility, authority, and accountability for planning, coordinating and evaluating the patient's care needs.
Provides nursing care to patients and families needing rehabilitation, medical intervention, advanced illness, or end-of-life care. Principle Responsibilities

Required Qualifications:
- Associate's or Vocational degree in Nursing
- 1+ year of Home Care or Hospice experience
Preferred Qualifications:
- Bachelor's degree in Nursing
Licenses/Certifications:
- Licensed Registered Nurse – MN Board of Nursing required if working in state of MN upon hire / WI Department of Safety & Professional Services required if working in the state of WI upon hire
- Valid Driver's License required upon hire - MN and WI residents must obtain a valid driver's license in their state of residence within 60 days of employment
- BLS Tier 1 - Basic Life Support - Multisource required by completion of orientation
- CHPN Certified Hospice and Palliative Nursing; or RN-BC RN Case Management; or ANCC-RNCM Certification in Case Management preferred

Pay Range:
$40.79 to $61.77 per hour
The pay described reflects the base hourly pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
