Position: RN – Home Care/Hospice (Full time 0.8 FTE; 8‑hour day/evening shifts 12:00 PM – 8:30 PM). No weekends, no holidays. Occasional on‑call. Mileage reimbursement at IRS rate.

Key Position Details

0.8 FTE (64 hours per two‑week pay period). 8‑hour day/evening shifts from 12:00 PM – 8:30 PM. No weekend or holiday shifts.

Responsibilities
  • Assessment – Gather, prioritize, synthesize comprehensive data; use evidence‑based assessment tools.
  • Diagnosis – Analyze assessment data to determine and document nursing diagnoses, share with team.
  • Outcomes Identification – Identify expected outcomes with patient/family; set realistic, measurable goals.
  • Planning – Develop individualized care plans that integrate evidence‑based practice and regulatory standards.
  • Implementation – Execute interventions safely and timely; coordinate activities; document interventions.
  • Evaluation – Assess patient progress, revise diagnoses, outcomes, and plans; involve all team members.
  • Quality of Practice – Participate in quality improvement; incorporate QI data to enhance outcomes.
  • Education – Stay current with nursing practice; engage in continuing education and apply new knowledge.
  • Professional Practice Evaluation – Conduct self‑evaluation, seek feedback, set professional development goals.
  • Collegiality – Share knowledge, give constructive feedback, foster a supportive environment.
  • Collaboration – Partner with patient, family, and interdisciplinary team to improve outcomes.
  • Ethics – Maintain professionalism, advocate for patients, perform ethical decision making.
  • Research – Incorporate best evidence and research findings into practice.
  • Resource Utilization – Plan and deliver care safely, effectively, and ethically with limited resources.
  • Leadership – Serve as role model, promote positive workplace, participate in shared decision‑making.
  • Environmental Health – Apply environmental health strategies; reduce risks for staff and patients.
  • Charge Nurse (when acting) – Coordinate unit operations, ensure efficient resource use, support staff.
  • Preceptor (when acting) – Design oriented learning, verify clinical competence, facilitate adjustment.
Qualifications

Required
:

  • Associate’s or Vocational degree in Nursing.
  • 1+ year of Home Care or Hospice experience.
  • Licensed Registered Nurse – MN Board of Nursing required if hired in MN; WI Department of Safety & Professional Services required if hired in WI.
  • Valid Driver’s License (MN or WI).
  • BLS Tier 1 – Basic Life Support required upon orientation.

Preferred
:

  • Bachelor’s degree in Nursing.
  • CHPN Certified Hospice and Palliative Nursing; RN‑BC; RN Case Management; ANCC‑RNCM Certification in Case Management.
  • Experience with on‑call travel and geographic mobility.
Physical Demands

Medium. Lifting up to 32 lbs. occasionally and up to 25 lbs. frequently.

Pay Range

$40.79 to $61.77 per hour. Rate may vary with experience, education, union agreement, shift differential.
