Key Position Details: .9 FTE (72 hours per two-week pay period); 8-hour day shifts;
Occasional weekends;
Travel to outreach sites required. Meet our Care Team Members:
Licensed Practical Nurse

Responsibilities
  • Provide patient care support as delegated by Provider or RN.
  • Administer therapies including medications, schedule follow-up visits (and prior authorization) or other support specific to a specialty provider.
  • Provide patient care pre/post encounter or procedure, preventative or other health information.
  • Collaborate with Provider and team on improving and managing quality goals.
  • May start, monitor, and discontinue IVs (dependent on site needs and training).
  • Room patients, manage patient flow and obtain adequate patient medical information/data.
  • Perform rooming and accurately measure patients’ vital signs and other indicators and record in medical record.
  • Coordinate communication of medical information and confirm information with patient when necessary, documenting per practice policy and standards.
  • Assist patients, as necessary, to prepare for physician exam, including providing psycho‑emotional support.
  • Assist in the collection, labeling and processing of specimens timely/accurately.
  • Assist physician with procedures including preparation of medications as required/needed.
  • Organize patient flow so physician can move efficiently from one room to the next.
  • Provide outbound calls to patients to follow up as directed, which may include notifying patients of test results.
  • Accurately and completely chart information in the patient medical record.
  • May checkout and schedule follow-up visits with provider or appropriate staff before patient leaves.
  • Receive or initiate patient phone calls; answer phones cordially and professionally in a timely manner.
  • Confirm or reiterate patient symptoms using patient’s own words and arrange for appropriate appointment or forwarding to RN or provider for review/recommendation.
  • Document all encounters and patient interactions accurately and completely in the electronic medical record.
  • Process physician orders and perform clinical functions within the scope of license as ordered by physician.
  • Respond promptly to patient needs and requests.
  • Review patient charts before visits to ensure test results and therapy reports are available; provide appropriate instructions as directed by physician.
  • Clean, prepare, organize and restock exam rooms; ensure maintenance and sterilization of instruments as trained.
  • Ensure teaching supplies are restocked and readily available; stock exam rooms according to standards and patient needs.
  • Other duties as assigned.
Qualifications
  • Required Qualifications:

    Associate's or Vocational degree from a Board of Nursing approved LPN program.
  • Licenses/Certifications:

    Licensed Practical Nurse MN BON or Licensed Practical Nurse Wisconsin (if working in Wisconsin or interacting with Wisconsin patients remotely). May require both state licenses based on services provided at the location hired.
  • Two years clinic LPN experience (preferred).
  • Must meet at least ONE of the following:
    Current BLS certification from the American Heart Association;
    Current BLS certification from the American Red Cross;
    Allina in-house BLS training (within 30 days of hire).
  • Physical Demands:
    Light work; lifting weight up to 20 lbs. occasionally, up to 10 lbs. frequently.
Pay and Benefits

Pay Range: $26.17 to $32.79 per hour. The pay described reflects base hourly pay range. Starting rate depends on factors including experience, education, and applicable union agreement. Shift/weekend differentials may apply to increase pay for certain shifts.
