We are seeking a full-time, in-person Internal Medicine Physician to join a mission-driven, multidisciplinary outpatient healthcare organization serving a diverse and medically underserved patient population.

Key Responsibilities Provide comprehensive outpatient internal medicine care to adult patients

Diagnose, treat, and manage acute and chronic medical conditions

Deliver preventive care, health screenings, immunizations, and wellness services

Develop and manage individualized treatment plans and follow-up care Collaborate with interdisciplinary clinical teams and coordinate specialty referrals

Deliver culturally competent care with consideration of social determinants of health

Participate in quality improvement initiatives, care coordination, and population health efforts

Accurately document patient encounters in the electronic health record (EHR)
Engage in clinical meetings, care team collaboration, and community health initiatives
