A growing healthcare services organization is seeking an experienced Medical Director to provide clinical leadership and oversight for a large network of skilled nursing facilities throughout the New York metropolitan area. The organization partners with skilled nursing facilities and senior care communities by providing specialized physiatry, pain management, and related clinical services directly within those facilities. Its physicians and healthcare professionals work alongside existing facility teams to deliver high-quality, patient-centered care focused on pain management, rehabilitation, mobility, functional improvement, and overall quality of life.
The organization has an established presence across Queens, Brooklyn, and select areas of Long Island and continues to expand its clinical operations and partnerships.
The Medical Director – Physiatry & Pain Management will serve as the senior clinical leader responsible for overseeing the delivery of high-quality physiatry and pain management services across approximately 30–40 skilled nursing facility locations. This individual will provide clinical direction, mentorship, leadership, and oversight to a team of approximately 10 on-site providers while ensuring consistency, quality, and excellence in patient care throughout the organization's network.
This is a highly visible leadership position that combines clinical expertise with provider management, quality oversight, and operational leadership. The ideal candidate will have substantial direct experience working within skilled nursing facilities and a deep understanding of the unique clinical, regulatory, and operational environment of SNFs. The Medical Director will primarily provide leadership and oversight remotely, with on-site travel required only when necessary to address clinical, provider, facility, or operational needs.
Candidates may be based in New York, New Jersey, or Connecticut, provided they maintain an active New York medical license and are able to travel to facilities throughout Queens, Brooklyn, and Long Island when needed.
- Provide overall medical and clinical leadership for physiatry and pain management services delivered across approximately 30–40 skilled nursing facility locations.
- Provide clinical oversight and management for a team of approximately 10 on-site physicians and advanced practice providers.
- Establish and maintain high standards of clinical care, ensuring providers consistently deliver evidence-based, patient-centered physiatry and pain management services.
- Serve as a senior clinical resource for providers managing medically complex patients, chronic pain, rehabilitation needs, functional limitations, and other conditions commonly encountered in the skilled nursing environment.
- Provide guidance on clinical decision-making, treatment planning, pain management strategies, rehabilitation goals, and coordination of care.
- Monitor provider performance and clinical quality across the organization's facility network, identifying opportunities for improvement and implementing appropriate corrective or development plans when necessary.
- Promote consistency in clinical practices, documentation, communication, and care delivery across multiple skilled nursing locations.
- Collaborate with facility administrators, Directors of Nursing, nursing leadership, rehabilitation teams, attending physicians, and other healthcare professionals to ensure effective coordination of patient care.
- Build and maintain strong working relationships with skilled nursing facility leadership and serve as a trusted clinical partner to facility teams.
- Support the organization's relationships with facility partners by ensuring a high level of provider performance, responsiveness, professionalism, and clinical service.
- Review clinical outcomes, quality indicators, documentation, and other relevant metrics to identify trends, risks, and opportunities to improve patient care.
