Job Summary:
The RN Nurse Assessor is responsible for conducting comprehensive in-home health evaluations and assessments to develop individualized long-term care plans. This role requires working in a hybrid environment of telehealth and in-field visits, ensuring high-quality patient care and accurate documentation.
Responsibilities:
- Perform initial in-home pre-evaluation of patients, assessing health status, strengths, care needs, and preferences.
- Review medical documentation and health referral forms to inform evaluations.
- Enter evaluation data into electronic systems and transmit as required.
- Document concerns, conflicting information, or issues identified during assessments.
- Communicate health care options, supports, vendor alternatives, and waiver options to consumers.
- Collaborate with interdisciplinary care teams to enhance care coordination and service delivery.
- Collect quality review data and document case notes within the CRM system.
- Identify opportunities for health promotion and illness prevention.
- Maintain comprehensive knowledge of community resources, payor requirements, and network services.
- Adhere to all position standards and participate in ongoing performance evaluations.
- Perform any additional duties as assigned by management.
Qualifications:
- Current RN licensure in New York State.
- Minimum of two (2) years recent experience in a home setting within the last ten (10) years.
- Experience conducting assessments similar to UAS or OASIS.
- Proven ability to perform comprehensive in-home evaluations and work independently.
- Familiarity with health documentation, case management, and patient care strategies.
- Strong communication, organizational, and interpersonal skills.
