Registered Nurse – Home Health

Home Health Nurse

Provide in-home assessment and care coordination for patients within a home health setting. Utilize nursing expertise to evaluate health status, develop care plans, and coordinate services to ensure optimal patient outcomes.

Responsibilities:

• Conduct comprehensive in-home health evaluations, including assessments of patient health, strengths, and care needs.
• Review medical documentation and health referral forms to inform assessments.
• Enter evaluation data accurately into electronic systems and transmit as required.
• Document concerns, conflicting information, and issues encountered during the evaluation process.
• Communicate care options, supports, and service vendor choices to consumers.
• Collaborate with interdisciplinary care team members to improve care coordination and service delivery.

Qualifications:

• Current RN licensure in New York State.
• Minimum of two (2) years of recent experience in a home health setting.
• Ability to perform assessments using UAS criteria and work with healthcare technology.
• Strong organizational, interpersonal, and communication skills.
• Demonstrated ability to work independently in a fast-paced, deadline-driven environment.
