Clinical Coordinator II/RN II

Reporting directly to Division Administrator the Clinical Coordinator II (Registered Nurse II), assists physicians with patient care, health education/promotion, electronic medical record (EMR) chart management, coordination of patient care and clinical protocols as needed. This role will work collaboratively with the non-invasive imaging and fetal cardiology team. The Clinical Coordinator II (Registered Nurse II) will work closely with multiple providers as well as other clinical and administrative staff to deliver an excellent patient experience that contributes to positive health outcomes for patients and a work culture that represents our values.

The candidate will be responsible for the following job duties:

  • Assesses and collects data related to patient health status and inputs this medical information into the EMR as instructed by the physician.
  • Works with the care team to develop the plan and care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) which includes assisting with medication management (e.g. medication renewals) inclusive of medical prior authorizations as needed. Administer immunizations and medications.
  • Under limited supervision, executes nursing interventions by collaborating with the care team and following standards of care established by the unit/department.
  • Supports on-going clinical functions of the service to provide compassionate patient-centered care.
  • Handle telephone, My Chart and in-person triage (for symptom management) as well as patient education.
  • Assist with management of urgent and emergent patient situations in conjunction with care team leadership. Address questions/concerns about the procedure(s)
  • Coordinates multidisciplinary health care services and arranges diagnostic tests (i.e. lab, x-ray) as ordered by physicians and performs diagnostic testing, therapeutic, and interventional procedures as needed under physician supervision.
  • Review and act on test results (lab, x-ray)
  • Review and manage communications from internal and external "customers" inclusive of inbasket messages, updating immunizations, updating health maintenance items.
  • Ensures infection control compliance within the practice.
  • Addresses appropriate measures to provide patient and family satisfaction.
  • May float to other clinics on as needed for operations need.
  • Coordinate and schedule inpatient and outpatient cardiac MRIs (CMRs) and cardiac CTs efficiently (i.e. assess length of scans based on protocols).
  • Coordinate scans with the 3

    Tower nursing team and/or anesthesia teams.
  • Coordinate combined Cath/MRI scans, including lymphatic imaging/interventions.
  • Serve as a liaison with referring physicians, particularly outside CHONY and with our adult CHD colleagues.
  • Maintain Cardiac MRI and CT databases; enroll patients for research studies; and assist with 3D Model Program.
  • Perform other related duties and responsibilities as assigned/requested.

May precept new clinical staff including Medical Assistants, LPNs and more junior nurses.

Minimum Qualifications

  • Requires a Bachelor's degree in Nursing or equivalent in education and experience.
  • 2-4 years related experience. Clinical rotation experience associated with BSN degree programs will be applied toward related experience requirements.
  • Current New York State RN License and BLS certified.
  • Ability to work indoors in an environmentally-controlled environment with a degree of dust, noise, odors, etc., without undue discomfort.
  • Ability to perform sedentary work that primarily involves sitting or standing for long periods of time.
  • Travel between clinical sites may be required depending on need.
  • Duties performed routinely require exposure to blood, bodily fluid, and tissue. Incumbent in expected to perform and/or/assist in emergency care and/or first aid as needed.

  • Employment is contingent upon successful completion of pre-offer and post-offer background checks in accordance with NYC's Fair Chance Act. Depending on the position, a drug screening and other verifications may be required. (University Policies).
  • Must successfully complete applicable compliance and systems training requirements.
  • HIPAA Privacy and Security Essentials Training
  • Contact with patients and/or human research subjects.
  • EPIC Access Required.
  • Ability to perform point of care testing; pend orders for providers per protocol.
  • Update the health record with historical items.
  • Prep clinical forms (school, work) for provider signature and precept new employees.
