Essential Responsibilities
Responsibilities include, but are not limited to:
- Supervise and assist direct support staff with personal hygiene, activities of daily living (ADLs), incontinence care, and maintaining a clean and safe environment.
- Oversee and participate in mealtime support and feeding protocols to ensure adherence to nutritional and medical guidelines.
- Administer medications in accordance with established procedures and regulations.
- Monitor and report infection control practices in compliance with organizational and site-specific exposure control plans.
- Ensure adequate first aid and medical supplies are available to maintain the safety and well-being of individuals supported.
- Document and report medical findings for RN review and inclusion in nursing care plans.
- Immediately report significant changes in an individual's medical condition to the RN Supervisor.
- Communicate, document, and follow up on medical concerns with healthcare providers and nursing leadership.
- Maintain communication with family members regarding pertinent medical updates and concerns.
- Provide routine, preventative, and post-operative nursing care and treatments as ordered by physicians or supervising nurses, including:
- G-tube feedings
- Wound and skin care
- Urinary drainage system maintenance
- Blood glucose monitoring
- Passive and active range-of-motion exercises
- Obtain and monitor baseline medical information, including vital signs and weight tracking.
- Ensure compliance with all applicable federal and state regulations related to residential healthcare services.
- Maintain accurate and timely medical documentation and charting.
- Attend medical appointments as requested and as scheduling permits.
- Follow established emergency procedures to ensure the safety and welfare of all individuals supported.
- Provide coverage in accordance with staffing schedules and operational needs.
- Attend and successfully complete all required training programs.
- Perform additional duties as assigned.
- Current New York State Licensed Practical Nurse (LPN) license required.
- High School Diploma
- AHA or equivalent CPR certification required.
- Experience working with individuals with intellectual and developmental disabilities preferred, but not required.
- Strong written and verbal communication skills.
- Ability to manage multiple priorities and work effectively as part of a team.
- Ability to interact professionally and compassionately with staff, individuals supported, family members, visitors, and external agencies.
- Ability to implement departmental goals, policies, procedures, and standards of care.
- Ability to oversee the proper use of supplies, equipment, and maintain a professional environment.
- Valid New York State driver's license strongly preferred.
