Job Summary:

We are seeking a dedicated Registered Nurse for home health assessments in the Bronx area. The ideal candidate will conduct comprehensive evaluations to support long-term care planning and coordinate with interdisciplinary teams to ensure quality patient care.

Responsibilities:

• Perform initial in-home assessments to evaluate patients' health status, strengths, care needs, and preferences.
• Review existing medical documentation and health referral forms relevant to each case.
• Enter evaluation data accurately into electronic forms and transmit as required.
• Document concerns, conflicting information, and other issues identified during assessments.
• Support continuity of care by reducing gaps and duplication in treatment plans.
• Communicate with consumers about healthcare options, support services, vendors, and waiver programs.

Qualifications:

• Valid RN licensure in New York State.
• Minimum of two (2) years recent experience conducting home health assessments within the last 15 years.
• Demonstrated ability to review health status questions and evaluate based on UAS criteria.
• Proficiency in using healthcare technology and electronic documentation systems.
• Strong organizational, interpersonal, and communication skills for effective multi-generational interaction.
• Ability to work independently in a fast-paced, deadline-driven environment.
