Registered Nurse – Home Health

Job Summary:

The RN Nurse Assessor in Home Health provides in-home evaluations to assess health status, care needs, and develop individualized long-term care plans. The role involves conducting hybrid assessments, including both telephonic and in-field visits, to ensure quality and continuity of care for homebound patients.

Responsibilities:

• Conduct initial in-home pre-evaluations assessing patient health, strengths, and preferences.
• Review medical documentation and health referral forms to inform assessments.
• Enter evaluation data into electronic systems and transmit as required.
• Document concerns or conflicting information identified during assessments.
• Collaborate with consumers and care teams to discuss health options and support services.
• Facilitate communication between the Call Center and eligible consumers for managed care enrollment.

Qualifications:

• Possess current RN licensure in New York State.
• Have at least two (2) years of recent home care experience within the last ten (10) years.
• Demonstrate experience conducting assessments similar to UAS or OASIS, including in the last 365 days.
• Show experience caring for geriatric populations and managing chronic diseases at home.
• Be able to work independently in a fast-paced, deadline-driven environment.
• Exhibit excellent organizational, interpersonal, and communication skills.
