Description:
Required licensure includes a current RN licensure in Client. Two years of experience in a home setting within the last ten years is necessary. This may include experience working for a Certified Home Health Agency or similar. Candidates must have recent experience conducting comprehensive assessments of patients. Preferred qualifications include experience conducting assessments in UAS-NY in the last year. Experience caring for a geriatric population and managing chronic diseases in the home setting is desirable.
A combination of paraprofessional home care experience and RN experience caring for patients is also preferred. Candidates will conduct initial in-home pre-evaluations of patients. They will evaluate health status, strengths, care needs, and preferences. The development of individualized long-term care service plans is essential. Candidates will review medical documentation relevant to each case. They must enter evaluation data into electronic forms and document any issues that arise.
Continuity of care will be emphasized to reduce fragmentation in treatment plans. Candidates will discuss healthcare options and required services with consumers. Communication will be initiated with eligible consumers for enrollment into managed care plans. Enhancing communication with interdisciplinary team members is crucial for care coordination. Quality review data will be collected to support outcome measurements. Candidates should identify opportunities for health promotion and illness prevention.
A comprehensive working knowledge of community resources and payor requirements is necessary. Candidates will meet all standards established for this position. Additional duties may be assigned as required.
