Registered Nurse – Home Health

Job Summary:

Provides comprehensive in-home nursing assessments and evaluations to support enrollment and care planning for community-based health programs. Ensures accurate data collection, patient engagement, and coordination with care teams to optimize health outcomes.

Responsibilities:

• Conduct initial pre-evaluation assessments of patients in their homes, including health status, strengths, care needs, and preferences.
• Review and interpret medical documentation and health referral forms relevant to each case.
• Record evaluation data accurately into electronic forms and transmit as required.
• Document any concerns or conflicting information encountered during assessments.
• Communicate health care options, supports, service vendors, and waiver options to consumers.
• Facilitate communication between the Call Center and eligible consumers for enrollment into managed care health plans.
• Collaborate with interdisciplinary care team members to improve care coordination and service delivery.
• Collect quality review data and support outcome measurements with proper documentation.
• Identify opportunities for health promotion and illness prevention to enhance patient wellbeing.
• Maintain current knowledge of community resources, payor requirements, and network services.

Qualifications:

• Possess current RN licensure in New York State.
• Have at least two years of recent experience in a home setting within the last fifteen years.
• Demonstrate proficiency in conducting comprehensive patient assessments and evaluations.
• Exhibit strong organizational, communication, and interpersonal skills.
• Ability to work independently in a fast-paced, deadline-driven environment.
• Comfortable working with technology and electronic data management systems.
