We are seeking a nurse interested in working in the community as an ACT Team member servicing individuals with behavioral health needs.

Manhattan, NY
Schedule:
Mon-Fri 09:00am-05:00pm Compensation:
Pay Range: $50.00-$55.00 Per HR
Role Overview :
As part of a mobile, multidisciplinary ACT team, the RN will provide comprehensive nursing care and support services to individuals with serious mental illness in community-based settings.
Key Responsibilities:
Conduct physical and health assessments Monitor ongoing health status and coordinate care Administer IM injections and manage medications as prescribed Provide psychiatric and substance use–related nursing support Deliver health education, daily living skills training, and medication management Coordinate medical treatment with primary care and community providers Support individuals in obtaining stable housing and community resources
Qualifications:
Active Registered Nurse (RN) license in New York State Strong communication and interpersonal skills Comfortable working independently in the community Proficiency with basic office software and internet research Please email:
