Position: Primary Care Nurse Practitioner / Physician Assistant

Location: Bronx, NY

Schedule: Full-Time

Job Summary

We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide primary care services to adult and senior patients. The ideal candidate will deliver compassionate, patient-centered care, manage acute and chronic conditions, and collaborate with healthcare professionals to support positive patient outcomes.

Responsibilities

• Conduct physical examinations and assess medical histories.
• Diagnose and treat acute and chronic health conditions.
• Order and review diagnostic tests and laboratory results.
• Develop individualized treatment and care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with healthcare professionals to coordinate patient care.
• Educate patients and families about treatment options and health management.
• Maintain accurate medical records and clinical documentation.

Requirements

• Active, unrestricted New York NP or PA license in good standing.
• At least 6 months of relevant clinical experience.
• Strong communication, clinical judgment, and critical-thinking skills.
• Commitment to compassionate, patient-centered care.
