Job Title: Level 2 Registered Nurse Case Manager
Location: NY
Pay: $2,834.80 per week
Shifts: Monday - Friday, Day shift 8:00am-4:30pm, weekends per need
Seven Healthcare are seeking a dedicated Level 2 Registered Nurse Case Manager to join a dynamic care team in NY. This position involves providing skilled nursing care and comprehensive case management in a home health care setting, ensuring an optimal work-life balance with scheduled daytime shifts.

- Certified Home Health Care Experience Required
- EPIC or Netsmart experience preferred
- OASIS experience
- Telemetry experience
- Current RN License (NY License ONLY)
- One year of experience as a clinical nurse in a hospital required
- Critical thinking skills and ability to make clinical judgments
- Strong communication and assessment skills
- Current NYS driver's license
- Basic Life Support (BLS) certification
