* Job Description:

** The Patient Care Coordinator will coordinate all aspects of the patients care allowing for seamless communication between the patients and all disciplines assigned to the case.
** What we are looking for:
*** High School Graduate or GED
* Two years recent work experience, preferably in home care
* Strong verbal skills and written communication skills.
* Good organizational skills.
* Good customer service skills
* Bilingual preferred
