Momentum at South Bay for Rehabilitation and Nursing is looking for a Full-time Admissions Coordinator.
- Good oral and written communication skills.
- Interview and assessment skills.
- Knowledge of entitlements and community resources.
- Knowledge of health care and gerontology issues.
- Maintain functions of the Admissions Department.
- Conduct tours and provide information about Momentum at South Bay to applicants, hospital discharge planners and community agencies.
- Admit residents in accordance with established policies and procedures.
- Upon admission, responsible to complete, review and explain admission documents to residents and/or sponsor obtaining signatures as necessary.
- Complete pre-admissions in Sigma EMR.
- Other related duties as assigned by Admissions Director
